About Harvard Medical School Center for Primary Care

Delivering the best, most efficient primary care is the greatest challenge facing medicine today, because primary care serves as the foundation of all medical care - both care delivered and care averted. More primary care physicians in communities is associated with improved mortality and has a unique capacity to improve health equity, yet a worsening physician shortage and overburdened workforce is inhibiting primary care from functioning at its highest capacity.

Given the field’s demonstrated value and critical importance, the Harvard Medical School Center for Primary Care was established in 2010 to elevate and build a primary care presence not only on the HMS campus, but among its local, national, and international communities. It fosters a dynamic community to promote and support primary care equity, resilience, and value to improve health and well-being. In addition to conducting some of the nation’s leading primary care research, the Center supports health care systems transformation, innovation, and education. It also facilitates collaborations between HMS, HSDM, and community-based organizations in the greater Boston area.

About the collection

While primary care leaders and practices have been chronicled in many venues, readers are limited to learning what these leaders or practices are doing, not how they operate. The Harvard Medical School Center for Primary Care seeks to fill this critical knowledge gap by profiling exemplar primary care practices, care delivery models, and health systems in order to learn how they deliver primary care.

The case studies in this collection aim to create and disseminate knowledge about best practices in primary care, explicate how to best structure primary care delivery, and identify how systems have organised to mitigate contemporary challenges including managing population health, controlling the high cost of care, and improving the patient and physician experience. 

The cases in the Primary Care Systems case collection are used to train medical students, medical residents, as well as primary care and inter-professional health staff, practitioners, administrators, and leaders. The cases are also appropriate for public health or management audiences seeking to better understand primary care and its fundamental challenges.

Free cases

In line with Harvard Medical School Center for Primary Care's mission to promote and support primary care equity, resilience, and value to improve health and well-being, they are pleased to make six of the cases in their collection available to all for free. Three of the cases are 'classics', foundational to understanding the complexity of primary care delivery, and three are more recent additions to the collection that were generously underwritten with grant support.
