This is the fourth of a four-case series (NAC1402 to NAC 1405). Was the ''Challenger'' disaster of January 1986 inevitable? By tracing its historical antecedents and documenting the final launch decision-making process, we have an opportunity to discover how the tragedy resulted from dynamics that are endemic to everyday organizational life.
